Symptoms
- Your phone showed a sign-in approval request you did not initiate
- You tapped "Approve" by mistake or out of habit
- You may see sign-in notifications from unfamiliar locations
Fix it yourself
- Change your company password immediately from a trusted device via the company portal.
- Deny any further MFA prompts you did not trigger yourself.
- Check your recent sign-in activity in your account portal if available, and note anything unfamiliar.
- Sign out of all active sessions if your account portal offers that option.
- Report the event to the service desk — someone likely has your password.
If that did not help
Open a ticket immediately with the time of the prompt, whether you approved it, and any unfamiliar locations shown in your sign-in history.
Note: Only approve MFA prompts that appear while YOU are actively signing in.
